I explain what is hiding behind the word 「reflux」 in a vein duplex ultrasound report, what duration of it is considered pathological, and what to do if there are no complaints but reflux is present.

Reflux on vein duplex ultrasound is a backward flow of blood through a valve that does not close in time. The term itself does not mean 「surgery is needed」: what matters is the specific vein, the duration of the backward flow, and whether symptoms are present. Let's go through the report point by point so the conclusion stops being a set of frightening letters.

What vein duplex ultrasound looks for

Duplex scanning shows how blood moves through the leg veins in real time. Normally, valves let blood flow toward the heart and then close, preventing it from flowing back down under gravity. If a valve does not close tightly, blood briefly flows in the opposite direction during certain movements — this is called reflux.

This is a direct check of how the valve apparatus is working. If you get a vein duplex ultrasound, the report will not just say 「varicose veins」 or 「normal」 — it will contain specific numbers and vein names, which determine the further approach.

GSV, SSV and the junction — decoding the abbreviations

The report almost always includes designations like these:

  • GSV — great saphenous vein, the largest superficial vein of the leg, running from the foot along the inner surface of the calf and thigh.
  • SSV — small saphenous vein, running along the back of the calf.
  • Saphenofemoral junction (SFJ) — the point where the GSV joins the deep femoral vein. Valve failure right here often triggers varicose transformation of the entire vein below.
  • Saphenopopliteal junction (SPJ) — the analogous point where the SSV joins the popliteal vein.

A note reading 「reflux in the GSV at the SFJ level」 means the problem starts at the very top valve of the great saphenous vein — the one that is supposed to hold back the column of blood from the thigh to the foot.

Vein diameter: why the number in millimeters matters

Besides reflux, the report specifies the vein diameter at several points — usually in the upper third of the thigh, at knee level, and on the calf. An enlarged vein is not a diagnosis but a measurement, though it shows how far the changes have progressed: a vein with reflux usually widens gradually, because the wall stretches under the increased pressure of the blood column.

Diameter should be assessed over time and together with reflux, not as a stand-alone number. The same figure can mean different things in different patients: for one it is varicose disease, for another it is simply a feature of vein structure with no clinical significance.

The Valsalva maneuver: how valves are tested

To provoke reflux and see it on the screen, the doctor asks the patient to bear down on exhalation with the glottis closed — this is the Valsalva maneuver. Pressure in the abdominal cavity is transmitted to the leg veins, and if a valve is incompetent, the sensor detects the backward flow of blood.

Compression tests produce a similar effect — quickly squeezing and releasing the calf with a hand or a cuff. Both methods are standard and do not cause discomfort, but they require the doctor's experience: too weak or too strong a load distorts the result.

What reflux duration is considered pathological

This is the key number in the report. For superficial veins (GSV, SSV and their tributaries), a backward blood flow lasting more than 0.5 seconds is considered pathological. Shorter reflux — for example, 0.2–0.3 seconds — is usually regarded as a physiological variant that does not require intervention.

For deep veins the threshold values are higher (usually from 1 second): their valves work differently and bear a greater load.

The figure 「0.6 seconds」 by itself is not a verdict: it is interpreted together with the vein's diameter, its course, the presence of tributaries with backward flow, and the clinical picture.

Reflux with no complaints — what to do?

Reflux is often found incidentally — during an ultrasound for another reason or at a preventive checkup, when the legs do not hurt and the veins look normal. This is a common situation: there is no need to panic, but the finding should not be ignored either.

A sensible course of action:

  • Do not delay a consultation with a phlebologist even if there are no complaints — the doctor will assess the risk of progression in your specific case.
  • Pay attention to symptoms that are easy to miss: heaviness in the legs by evening, swelling, night cramps — they may be related to reflux even without obvious varicose veins.
  • Repeat the ultrasound after an interval determined by the doctor, if the decision is to observe rather than treat right away.
  • Do not start taking venotonics on your own or wearing compression stockings 「just in case」 without understanding why it is needed in your specific situation.

Asymptomatic reflux of short duration in a single tributary is not the same as pronounced reflux in the GSV trunk with vein enlargement and complaints. The approach in these cases differs and is determined by the doctor during an examination.

Which report calls for which specialist

Not every mention of reflux requires immediate treatment, but there are situations where a visit to a phlebologist in the near future is advisable:

  • Reflux in the GSV or SSV trunk lasting significantly more than 0.5 seconds, especially with vein enlargement.
  • Visible, tortuous, bulging veins — bulging veins — together with reflux on ultrasound.
  • Induration, redness or pain along the course of a vein — here it is important to rule out thrombosis, and you should seek care urgently rather than as a routine matter.
  • Skin changes on the calf: darkening, thinning, and especially a non-healing wound — this may be related to chronic venous insufficiency, and it is worth paying attention to the topic of trophic ulcers.
  • Reflux separate from the trunk, feeding spider veins: here the issue is not varicose disease but spider veins, which have their own approach.

If reflux is confirmed and the doctor recommends treatment, there are several options today: endovenous laser ablation — EVLA — for trunk veins with reflux, sclerotherapy for tributaries and individual nodes, miniphlebectomy for removing varicose nodes through punctures, and the CLaCS method — a combination of transdermal cryo-laser and a sclerosant injected into the feeding vein — for spider veins and small networks rather than large trunks. Which method is suitable depends on what the ultrasound showed and is decided individually.

Why ultrasound performed by an operating phlebologist is more informative

Vein duplex ultrasound is not just a technical procedure of 「turn on the machine — get numbers」. Interpreting reflux requires understanding how this finding will affect the decision: to observe, to suggest compression, or to discuss intervention. When the ultrasound is performed by a doctor who treats venous pathology themselves, they look at the picture with an eye toward tactics — which veins are the source of the problem and which are secondary, whether progression should be expected, and what scope of intervention is actually needed.

This does not mean that an ultrasound report from a diagnostic physician is worse in terms of formal numbers. But combining 「diagnosis plus treatment with the same specialist」 reduces the chance of discrepancies between the report and what happens at the follow-up visit.

If reflux is only 0.6 seconds — is that already serious?

A figure slightly above the 0.5-second threshold does not by itself indicate the severity of the process. What matters is which vein the reflux is in, and whether there is enlargement and symptoms. This is worth discussing with the doctor at an in-person consultation with the report in hand.

Can reflux disappear on its own, without treatment?

Changes in the valve apparatus usually do not recover on their own. The course can be slow and may cause no symptoms for years, but one should not count on reflux disappearing spontaneously — instead, the dynamics are monitored with ultrasound.

Do I need a vein duplex ultrasound if my legs do not hurt?

If there are visible vascular changes, heaviness by evening, swelling, or cases of varicose veins among close relatives, it makes sense to get an ultrasound even without pronounced pain — reflux at an early stage often has no symptoms.

How is vein duplex ultrasound different from a regular vascular ultrasound?
