What Is Venous Thrombosis
A thrombus is a blood clot that forms inside a vein and obstructs normal blood flow. Depending on where it develops, there are two fundamentally different conditions.
Superficial vein thrombophlebitis is thrombosis and inflammation of a subcutaneous vein, often against a background of varicose veins. It is visible and palpable: a painful, firm cord under the skin, accompanied by redness and local warmth. On its own it less often poses a life-threatening risk, but it can extend into the deep system.
Deep vein thrombosis (DVT) is far more serious. It develops in the deep veins of the leg, frequently without obvious external signs, and it is DVT that carries the risk of pulmonary embolism (PE). Symptoms: How to Tell Them Apart and When to Act
Superficial Vein Thrombophlebitis
- A painful, firm cord along the course of a visible vein
- Skin redness over the vein and localized warmth
- A hardened area you can feel with your fingers
- Often occurs against a background of pre-existing varicose veins
Deep Vein Thrombosis
- Sudden swelling of one leg — noticeably larger than the other
- A bursting pain in the calf or thigh that worsens with walking
- Skin that feels warm to the touch, ranging in color from reddish to bluish-purple
- Sometimes — more prominent superficial veins on the affected leg

Causes and Risk Factors
Immobility
Long flights or car journeys (4–6 hours or more), bed rest, and casts cause blood to pool and clot more easily.
Surgery and Injury
Major procedures, fractures, and leg trauma sharply increase the risk of thrombosis in the postoperative period.
Hormones
Hormonal contraception and hormone replacement therapy raise blood clotting tendency.
Pregnancy and Childbirth
Increased clotting and compression of pelvic veins — especially in the third trimester and the first weeks after delivery.
Cancer and Thrombophilia
Tumors, their treatment, and inherited clotting disorders are significant risk factors.
Varicose Veins, Age, and Weight
Advanced varicose veins, age over 60, excess weight, and smoking all increase the likelihood of thrombosis.
From Thrombophlebitis to Pulmonary Embolism: How the Danger Escalates
It helps to think of venous thrombosis as a scale of rising threat. The goal is to stop the process as early as possible and prevent it from moving to the next level.

Superficial Thrombophlebitis
Thrombosis of a subcutaneous vein. Painful and visible, but with timely treatment it is generally not life-threatening.

The clot extends upward toward the junction with the deep vein — risk of progression to DVT, requiring proactive management.

A clot within the deep venous system — a potential source of pulmonary embolism. Always an urgent situation.

A fragment of the clot breaks off and travels to the pulmonary artery. A life-threatening emergency requiring immediate care.
The level is determined by the doctor using ultrasound. The key point is not to 'wait out' the symptoms: the sooner the diagnosis is confirmed, the easier it is to stop the clot and prevent complications.
Why Untreated Thrombosis Is Dangerous
The danger of thrombosis is not always in the leg itself. That is precisely why it cannot be 'waited out at home.'
Pulmonary Embolism
The most serious complication: a clot fragment travels through the bloodstream to the lungs. It can range in severity — up to and including life-threatening.
Post-Thrombotic Syndrome
A long-term consequence of DVT: chronic swelling, heaviness, skin changes, and the risk of venous ulcers developing months or years later.
Spread of Thrombosis
Without treatment, the clot grows and can extend from the superficial into the deep venous system.
Recurrence
After a thrombotic episode, the risk of a repeat event is higher without ongoing monitoring and preventive care.
Diagnostics: Urgent Vein Duplex Ultrasound
The primary diagnostic tool is vein duplex ultrasound: within 20–30 minutes it reveals the clot, its boundaries, the mobility of its head, and the degree of vein occlusion. A D-dimer blood test may be added when needed. Ultrasound answers the key questions: where the clot is, how dangerous it is, and what course of action is required. When thrombosis is suspected, I do not postpone the examination.

Treating thrombosis is not a matter of 'choosing a method' the way varicose vein treatment is. Here, the foundation is correctly selected medication therapy, an appropriate activity regimen, and ultrasound-guided monitoring. The approach is always individualized and depends on the type, location, and age of the clot.
Anticoagulants — The Foundation of Treatment
The primary goal is to stop the clot from growing and prevent it from fragmenting. This is achieved with anticoagulants (blood-thinning medications) administered on a strict schedule, combined with compression therapy and a guided activity plan. Dosages and duration are tailored to each patient and require monitoring — self-treatment here is both inappropriate and dangerous. In some cases — for example, ascending thrombophlebitis near the saphenofemoral junction — surgical intervention or inpatient care may be needed.

Myths About Thrombosis
Myth:The clot will dissolve on its own — just wait it out
The Reality:The body does partially break down clots, but relying on this is risky: a fresh clot can grow and break free. Without treatment, the risk of pulmonary embolism and post-thrombotic syndrome is higher.
Myth:You should warm and massage the affected leg
The Reality:Absolutely not. Heat and mechanical pressure can dislodge a clot. When thrombosis is suspected, the leg must not be massaged, rubbed, or exposed to heat.
Myth:Thrombophlebitis is not serious — it's just vein inflammation
The Reality:Superficial thrombophlebitis can extend into the deep venous system, especially if the clot is ascending. That is why it also requires ultrasound assessment, not just a topical ointment applied at random.
Myth:After thrombosis you must not move
The Reality:Strict bed rest has long since ceased to be the standard. With properly managed treatment, controlled walking is generally permitted and even beneficial — but the regimen is determined by the doctor after a thorough diagnostic assessment.
Thrombosis Prevention
- On long flights — get up, move your feet, stay hydrated, and wear compression stockings if you are at risk
- After surgery or injury — follow your prescribed thrombosis prevention plan
- Treat varicose veins: advanced vein disease increases the risk of thrombophlebitis
- Discuss hormone therapy with your doctor if you have risk factors
- Stay active, maintain a healthy weight, and avoid smoking
- If you have a hereditary tendency toward thrombosis — get screened and stay under medical supervision

What is the difference between thrombophlebitis and deep vein thrombosis?
Thrombophlebitis is thrombosis of a superficial, subcutaneous vein: it is visible and painful, but less often life-threatening. Can I walk with thrombosis?

Anywhere from several weeks to several months or longer — depending on the type of thrombosis, its underlying cause, and the risk of recurrence. Only a doctor can determine the regimen and duration; stopping or changing the medication on your own is not safe.
Can thrombosis come back?
Yes, there is a risk of recurrence, especially if underlying risk factors remain. That is why follow-up care, repeat ultrasound checks, and preventive measures are important after an acute episode — they reduce the chances of another event and of post-thrombotic syndrome.
